Resident / Student Orientation Information

To facilitate in-processing at the Bay Pines VA Healthcare System, we request that you complete the required paperwork and mandatory training prior to your arrival at orientation. Please follow the steps 1-7 detailed below (1-2 must be completed a minimum of 6 weeks prior to your rotation)

Step 1: 

Complete VHA Mandatory Training for trainee (instructions below)

VHA Mandatory Training for Trainee (MTT)

Step 2:

Complete the respective attached checklists (Resident Checklist or Student Trainee Checklist) and a copy of your current driver’s license or identification card and fax it to secured fax number:(727) 398-9556.
Note: Instructions to fill out the forms are attached below. Ensure you have the most recent version of Adobe Reader to your computer before downloading the paperwork to fill out. You can download it here: https://get.adobe.com/reader/. All forms must be completed and signed electronically (instructions below).
Note: Documents should be downloaded to your desktop prior to electronically signing any of the forms listed below.

Step 3:

Schedule your fingerprint appointment by visiting the Veterans Affairs (VA) personal identity verification (PIV) Card Appointment Reservation Application at https://va-piv.com/#maincontent.

Step 4: (Optional)

Step 5:

Residents must have a National Provider Identification Number. Follow the instructions below if you do not have one.

Step 6:

Complete the VA Learners’ Perception Survey at the end of your Bay Pines VA Healthcare System rotation at https://www.va.gov/oaa/surveys/. Information from this survey provides feedback to identify areas of excellence as well as areas needing improvement survey should take about 10 minutes to complete.
