Resident / Student Orientation Information

To facilitate in-processing at the Bay Pines VA Healthcare System, we request that you complete the required paperwork and mandatory training prior to your arrival at orientation. Please follow the steps 1-7 detailed below (1-2 must be completed a minimum of 6 weeks prior to your rotation)

Step 1: 

Complete VHA Mandatory Training for trainee (instructions below)

Step 2:

Complete the respective attached checklists (New Resident Physician Checklist, Resident Renewal Checklist, or Student Trainee) and a copy of your current driver’s license or identification card and email it to vhabayacademicaffiliations@va.gov.
Note: Instructions to fill out the forms are attached below. Ensure you have the most recent version of Adobe Reader to your computer before downloading the paperwork to fill out. Step 3:

Schedule your fingerprint appointment by visiting the Veterans Affairs (VA) personal identity verification (PIV) Card Appointment Reservation Application at https://va-piv.com/#maincontent.
Once fingerprint appointment has been completed, log back into the VA PIV Card Appointment Reservation Application and schedule an appointment to pick up your PIV badge1-3 days prior to rotation start date.
Note: Please do not schedule this appointment any sooner as your badge will not be ready. If you have not received an email with and orientation time, please contact academic affiliation at 727-398-9533 or email vhabayacademicaffiliations@va.gov

Step 4: (Optional)

Highly recommend but not required Computerized Patient Record System (CPRS)Training link is available at:

Step 5:

Residents must have a National Provider Identification Number. Follow the instructions below if you do not have one.

Complete the VA Learners’ Perception Survey at the end of your Bay Pines VA Healthcare System rotation at https://www.va.gov/oaa/surveys/. Information from this survey provides feedback to identify areas of excellence as well as areas needing improvement survey should take about 10 minutes to complete.
