Resident / Student Orientation Information

Read and review the appropriate documents then print and complete the required forms.  Please report to Bldg 20, Room 206 at 8:00 am on your first day of rotation unless otherwise instructed by Academic Affiliations staff.  If you are a medical resident and your first day falls on the weekend, you must call 727-398-9533 or 1-888-820-0230 ext. 5033, to schedule your orientation prior to your first day so that you will be cleared and ready to begin working on the weekend.  Please bring all completed forms and training certificates with you to orientation.

All Residents and Students assigned to the Bay Pines VA Healthcare System must be fingerprinted FOUR (4) weeks prior to starting their rotation.

FINGERPRINT APPOINTMENTS:  Please call 727-398-9533 or 1-888-820-0230 ext. 5033 to set up an appointment for fingerprinting four (4) weeks prior to the beginning of your rotation.  If you reside in the local area please have several dates/times in mind before you call. 

Available appointments are Monday through Friday
8:30 am – 3:30 pm.

You will need two acceptable forms of identification.  Please refer to the PIV Proofing Criteria sheet for details. 

If you do not reside in the local area, please call us for detailed information on how to complete this process at a VA facility near you.

If you were already fingerprinted at any VA facility within the last 12 months, you still need to contact us so that we may verify this information otherwise you will have to be re-fingerprinted.

This survey asks you to rate various aspects of your VA clinical training experience.  Information from this survey will provide feedback for identifying areas of excellence as well as areas needing improvement.  Please base your answers on the VA facility where you have had your most recent clinical experience.  To take this survey, you must have had clinical training at a VA facility during this academic year.  Please allow 10 minutes to complete this survey.  Go to: www.va.gov/oaa  then choose “Learners Perception Survey” in the middle of the page.
