Care Coordination Home TeleHealth (CCHT) allows veterans and caregivers to have personalized care and take a more active role in their healthcare treatment. It also provides a link to your healthcare team. CCHT provides in-home monitoring services to eligible veterans enrolled in Bay Pines VA.

The Bay Pines CCHT Program utilizes telemonitors (video and non-video), such as  messaging devices, for in-home monitoring; providing local and distant veterans easier access to health care. Through the use of the telehealth tools you are better prepared to manage your health. The services provided by Care Coordination Home TeleHealth (CCHT) offer veterans and / or the caregiver a daily link to their treatment team and their VA provider, thus improving the veteran's overall health and treatment of chronic illnesses.  The CCHT program is capable of monitoring chronic conditions such as high blood pressure, diabetes, chronic lung conditions, heart failure, chronic pain, or any combination; providing "The right care, in the right place, at the right time!"

The Care Coordination Office is located on the Bay Pines campus:
Nursing Home Care Unit-A / Building #101, Room A-12. You may find ample parking in the patient parking lots #8 (A & B) or you can choose to take one of the many hospital shuttles that will drop you off directly at the entrance to Building # 101.  We can reached  at: 727-319-1262 or 727-398-6661 Ext 1262 during regular business hours: Monday through Friday, 8:00 am until 4:30 pm.  

We also have offices located at Fort Myers VA Outpatient Clinic.  The telephone number there is 239-939-3939, Ext 6388.
