Clinical Preceptors - Pharmacy Residency Program

Paul V. Laucka, Pharm.D., MBA, is Assistant Chief, Pharmacy Service. He holds academic appointments as Clinical Associate Professor at the University of Florida, Nova Southeastern University College of Pharmacy, and Mercer University School of Pharmacy. Dr. Laucka received his B.S. in Biology from Northeastern University, B.S. in Pharmacy from Massachusetts College of Pharmacy, his MBA from Suffolk University, and his Doctor of Pharmacy from the University of Florida.

Gary Wilson, B.S., Pharm.D., is the Administrative Program Manager for Pharmacy Service. He is responsible for special projects and initiatives such as JCAHO preparedness. Dr. Wilson was a member of Bar Code Medication Administration V.A. National Workgroup for Versions 1 and 2 of BCMA. He is now the BCMA coordinator for the medical center. He coordinated the beta testing of the software at Bay Pines as well as the implementation of the released product to the field. Dr. Wilson was a member of the V.A. National Training Team for BCMA Versions 1 and 2. He is a mentor for St. Petersburg Junior College Cooperative Educational Program "School to Work Program" and a member of Pinellas Technical Education Centers Pharmacy Advisory Committee. He is also past president and past member of Board of Directors for Pinellas Pharmacy Association. Dr. Wilson received his B.S. in Pharmacy from Samford University and Pharm.D. from the University of Florida.

Carolyn Stephens, Pharm.D., is the Residency, Education, and Clinical Coordinator for Bay Pines VAHCS Pharmacy Service. She holds academic appointments at Florida A&M University, Mercer University, Nova Southeastern University, and the University of Florida. Dr. Stephens received her Doctor of Pharmacy from the University of Florida and completed a Primary Care Residency at James A. Haley Veterans Hospital in Tampa in 1992. From 1997 to 2002, she served on national VA advisory boards reviewing the role of clinical pharmacy specialists, advanced practice nurses, and physician assistants in Primary Care. During 2000 to 2001 she served as the advisory board chair and reported to the director of patient care for Veterans Health Services. In 2001 she established the charter Tampa Bay chapter of the Federal Pharmacist Association. Since 2003 she has been a co-coordinator of the Tampa Bay Residency Forum which includes all the pharmacy residency programs in the Tampa Bay area.

Eddie Grace, Pharm.D., is the program director for the Infectious Disease residency program (PGY-2), and Infectious Disease clinical pharmacy specialist at Bay Pines VA Healthcare System. Dr. Grace is the clinical pharmacy specialist for the pharmacy HIV/AIDS clinic and Hepatitis-C Clinic. He is also responsible for antibiotic surveillance and serves on the infection control committee. Dr. Grace also precepts the PGY-1 program residents during their infectious disease rotations. He holds academic appointments as Clinical Associate Professor at the University of Florida and Nova Southeastern University College of Pharmacy. Dr. Grace received his Doctor of Pharmacy (Pharm.D) from the University of Florida in 2005, and completed a primary care specialty residency at Bay Pines VA Healthcare System in 2006.

John Hoeldtke, B.S., Pharm.D., BCPS, is the program director of the PGY1 Residency Program (formerly Pharmacy Practice) and Clinical Pharmacy Specialist in the Pharmacy Investigational Drugs service. Dr. Hoeldtke received his B.S. in Pharmacy from the University of Georgia and his Doctor of Pharmacy from the University of South Carolina. He holds academic appointments as Clinical Associate Professor at the University of Florida, Nova Southeastern University College of Pharmacy, and Mercer University School of Pharmacy, is board certified as a Pharmacotherapy Specialist and is a member of the Investigational Review Board at Bay Pines VAHCS.

Valerie Stanard, PharmD, CDE, CLS, is the program director for the PGY2 Ambulatory Care Residency in Diabetes Management and Clinical Pharmacotherapy Specialist in Endocrinology at Bay Pines VAHCS.  Dr. Stanard received her Doctor of Pharmacy degree from the Medical College of Virginia/Virginia Commonwealth University in Richmond, Virginia in 2001.  She completed a post-doctoral residency in Diabetes and Cardiovascular Risk Reduction at the McGuire VA Medical Center in Richmond, Virginia before becoming a member of their Diabetes Health Center staff.  At the Richmond VA Medical Center, Dr. Stanard was a Clinical Research Coordinator and provider in the Diabetes Health Center, as well as Affiliate Assistant Professor for the MCV/VCU School of Pharmacy.  Dr. Stanard is Board Certified in Lipid Management and is a Certified Diabetes Educator.

Robert D. Stewart, Pharm.D., is the program director of the PGY1 Primary Care Residency Program at Bay Pines VA campus. He is a Clinical Pharmacy Specialist in the PharmCare clinic, which is a pharmacist-managed disease-state management clinic. He is also the clinical pharmacy specialist for Home Based Primary Care and the HAZ-MAT/EMRT team. He holds academic appointments as Clinical Associate Professor at Florida A&M University, University of Florida, Nova Southeastern University College of Pharmacy, Palm Beach Atlantic, and Mercer University School of Pharmacy. Dr. Stewart received his PharmD. from Florida A&M University College of Pharmacy and Pharmaceutical Sciences in 1996 and completed a Pharmacy Practice residency a Bay Pines VAHCS in 1997.

Sondra Adkinson, Pharm.D., C.G.P, FAAPM, is a Clinical Pharmacy Specialist in Primary Care and Pain Management and serves as program co-coordinator for the Pain Team. Dr. Adkinson received her B.S. in Pharmacy from Ohio Northern University in 1980 and her Doctor of Pharmacy from Nova Southeastern University in 1998. She is board certified as Geriatric Pharmacist specialist by examination in 1998 and 2003. She is also a credentialed pain practitioner by examination in 2004 recognized as a Fellow of the American Academy of Pain Management. She is also a certified Consultant Pharmacist for the State of Florida since 1986. She has been the recipient of an APHA Innovation Practice Grant and presented a paper and lecture at the San Antonio Annual Meeting 1999 on Pharmaceutical Care and the Visually Impaired Veteran. She is actively involved in the Pain Team at Bay Pines VAHCS and currently practices in PharmCare and the Chronic Pain Clinic. She is an active member of ASHP, APHA, ASCP, ACCP, AAPM, FSHP, FPA, SWFSHP PPA, and FPI.

Marie Beadle, PharmD, CGP, is a clinical pharmacist at the Port Charlotte VA Community Based Outpatient Clinic.  She received her Doctor of Pharmacy from the University of Florida in 2004, and completed a Geriatrics Pharmacy Practice residency at the North Florida/South Georgia VA Health System, Gainesville, Florida , in 2005.  She actively holds a license as both a Registered Pharmacist and Consultant Pharmacist in the State of Florida .  In 2007, she obtained certification as a Geriatric Pharmacist (CGP) with the Commission for Certification of Geriatric Pharmacists.  Her current position duties are divided between the Port Charlotte VA Home Based Primary Care Program (HBPC) and the Outpatient Clinic.  She is responsible for quarterly medication regimen reviews of all HBPC patients and participates in HBPC Interdisciplinary Team Meetings on a weekly basis.  In the primary care setting, she assists with anticoagulation management, disease state management (mostly hyperlipidemia), formulary initiatives, mail order processing, drug information procurement for facility providers, and patient education.

Hetal Prabodh Bhatt, Pharm.D., is an Ambulatory Care Clinical Pharmacist with a primary responsibility in the Pharm Care clinic, emphasizing control of patient's diabetes mellitus, hypertension, hyperlipidemia, anticoagulation, and compliance. Dr. Bhatt received her Bachelors of Science in Microbiology in1999 and Doctorate of Pharmacy in 2003 from the University of Florida. Dr. Bhatt completed a post-doctoral Pharmacy Practice Residency with an Emphasis in Ambulatory Care from the VA Chicago Health Care System in June 2004. Her residency research project was an Evaluation of a Newly Initiated Interdisciplinary Pain Clinic at the VA Chicago Health Care System. Her academic affiliations included Clinical Assistant Professor, Pharmacy Practice thru University of Florida, serving as an assistant clinical clerkship preceptor in Ambulatory Care. Dr. Bhatt is actively involved in the clinical training of Pharm.D. student and residents at the Bay Pines VAHCS and serves on the Residency Advisory Committee.

Michael Chaffman, B.S., Pharm.D, BCPS, is the Pharmacy Clinical Coordinator; he provides drug information and clinical decision support to pharmacy staff, oversees formulary management and pharmacoeconomic activities, and supervises drug procurement for the Bay Pines Healthcare System. Dr. Chaffman initiated clinical pharmacy support to the Primary Care Clinic area in 1985 and practiced in that arena (primarily in Anticoagulation) until 2004. He holds academic appointments at Florida A&M University, University of Florida and Nova Southeastern University. Dr. Chaffman received his B.S. in Pharmacy from Auburn University, and his Doctor of Pharmacy from Idaho State University. He is board certified in Pharmacotherapy . Prior to joining the Pharmacy Service at Bay Pines VAHCS, Dr. Chaffman was an Associate Editor for the Australasian Drug Information Service (ADIS Press Ltd.) in Auckland, New Zealand. He has authored a number of published drug monographs and therapeutic reviews and is a member of the American College of Clinical Pharmacy.

Dawn Dunham  Pharm.D., is the  Clinical Pharmacy Specialist for the Hepatitis C Clinic as well as the Clinical Pharmacy Specialist for the Neurology Team at Bay Pines VAHCS. Dr. Dunham received her Doctorate of Pharmacy in 1999 from Nova Southeastern University and completed a post-doctoral Primary Care  Residency from Bay Pines VAHCS. Dr. Dunham is involved in the clinical training of Pharm.D. student/residents  and has served as sub-investigator for research projects. Her previous practice areas include diabetes, lipid, HTN and anticoagulation management.

Denelle Lynn Hall, B.S., CACP, is a clinical pharmacist at the Manatee (Ellenton) CBOC.  She received her Bachelor of Science in Pharmacy from Wayne State University, Detroit MI, in 1995.  In 2007 she became a Certified Anticoagulation Care Provider.  Her responsibilities include anticoagulation management, disease state management (primarily hyperlipidemia), pharmacy initiative goals, customer education, tracking pharmacy outcomes and cost avoidance.

Kim Hall, Pharm.D., is a Clinical Pharmacist with practice responsibilities in the Pharm-Pain, Falls, Anticoagulation, and PharmCare clinics at Bay Pines VAHCS. Dr. Hall received her Bachelor's degree in Psychology from the University of Missouri-Columbia and Doctor of Pharmacy from the University of Missouri-Kansas City. She completed a post-doctoral pharmacy residency in Primary Care at Bay Pines VAHCS, where her residency project evaluated the effects of conversion to a Lantus (glargine) insulin regimen on blood glucose control and cost. Primary practice and research interests include pain management, diabetes, hyperlipidemia, renal disease, and nutrition. She is a member of ASHP and APHA.

Tod J. Hammill, B.S., Pharm.D., is the Clinical Pharmacy Specialist in Oncology/Hematology. He holds an academic appointment as Clinical Instructor at the University of Florida and has acted previously as a preceptor for Ferris State University, Nova Southeastern University College of Pharmacy, Mercer University School of Pharmacy and Florida A&M. Dr. Hammill received his B.S. in Pharmacy from Ferris State University and his Doctor of Pharmacy degree from University of Florida. His previous pharmacy experience also includes Home Infusion therapy and Long Term Care. His primary interest also includes pain management, hospice and enrollment of patients in chemotherapy clinical trials at the Medical Center.

Keri Justice, Pharm.D., is the CBOC (Community Based Outpatient Clinic) Pharmacy Supervisor and a Clinical Pharmacist at the Manatee and Sarasota VA Primary Care Clinics (CBOCs).  She holds academic appointments at University of Florida, Nova Southeastern University, Palm Beach Atlantic University-Lloyd Gregory School of Pharmacy and Ohio Northern University.  Dr. Justice received her Doctor of Pharmacy degree from Nova Southeastern University in 1995.  After graduating, she completed a Pharmacy Practice Residency at Bay Pines VAHCS in 1996.  In 1997, she started at the first Bay Pines CBOC in Sarasota.  Since that time, the CBOCs have grown to 7 clinics staffed with 8 clinical pharmacists following the model developed at Sarasota. Dr. Justice’s responsibilities include anticoagulation services, disease state management, including hyperlipidemia, diabetes mellitus and hypertension and conducts New Patient Orientation Group classes in Manatee and Sarasota.  She also completed the VISN 8 Competency Development for Leaders in the 21st Century program.  Dr. Justice is a member of FSHP and a full member of the American Academy of Clinical Pharmacists.

J. Daniel Justino, B.S., Pharm.D., BCPP, is the Clinical Pharmacy Specialist for Psychiatric Pharmacy. Dr. Justino received his B.S. from Northeastern University in Boston and his Doctor of Pharmacy from the University of Illinois at Chicago. He holds academic appointments as Clinical Assistant Professor in Psychiatric Pharmacy at the University of Florida, Florida A & M University, Nova Southeastern University College of Pharmacy, Northeastern University College of Pharmacy, and Mercer University School of Pharmacy. He has served on, both the IRB and R&D Committees, as well as been, the pharmacy representative to the Haz-Mat team. He is board certified in Psychiatric Pharmacy by the Board of Pharmaceutical Specialties of the APHA. Research includes new drug investigations, compliance strategies, and Group Process Medication Education Methods. He is an active member of CPNP, ASHP, FSHP, and SWFSHP.

Jacqueline Martinez, Pharm.D., is a Clinical Pharmacist. Dr. Martinez received her Doctor of Pharmacy from Nova Southeastern University and completed a Pharmacy Practice Residency at Bay Pines VAHCS. She previously completed a research project in Evaluation of Pharmacokinetic Parameters of Gentamicin in Patients Above and Below Ideal Body Weight. Her clinical practice interests include Internal Medicine, HIV/AIDS, and women's issues.

Megan R. Martinez, Pharm.D., is a clinical pharmacist with primary responsibilities in the Home Based Primary Clinic, Anticoagulation Clinic, and PharmCare Clinic, managing patients with hypertension, hyperlipidemia, diabetes, compliance, and Coumadin monitoring issues. Dr. Martinez received her Doctorate of Pharmacy at West Virginia University in May 2001. She completed a post-doctoral Pharmacy Practice Residency at Tampa General Hospital affiliated with Florida A&M University in June 2002. She completed a research project on weight based heparin dosing during her first residency. She later went on to complete her specialty residency in Primary Care at Bay Pines VAHCS in June 2003. She completed a research project on urinary cultures and sensitivities in different hospital settings during her second residency. Dr. Martinez is involved in the clinical training of Pharm.D. students and residents at the Bay Pines VAHCS. She also facilitates pharmacy residents in their research projects as a sub-investigator. Her academic affiliations include University of Florida, Mercer University, Florida A&M University, NOVA Southeastern University, Palm Beach Atlantic University and Ohio Northern University. Her primary practice areas of interest include diabetes, hyperlipidemia, hypertension, and anti-coagulation.

Cecilia C. Morelli, B.S., BCNSP, is the Clinical Pharmacy Specialist on the Nutrition Support Team. Also, she provides Pharmacy support to the Visually Impaired Service Team, arrangement of home IV infusions, and serving on various committees. She holds academic appointments as a Clinical Instructor at the University of Florida College of Pharmacy, Nova Southeastern College of Pharmacy, and Mercer University School of Pharmacy. Mrs. Morelli received her B.S. in Pharmacy from Union University, Albany College of Pharmacy. She is a board certified Nutrition Support Pharmacist. She is a member of the American Society for Parenteral and Enteral Nutrition and the Federal Pharmacists Association-Tampa Bay Chapter.

Michael W. Osborne, B.S., Pharm.D., is the Clinical Pharmacist for the Ambulatory Surgery Unit. Dr. Osborne received his B.S. in Chemical Engineering from Gannon University, B.S. in Pharmacy from Albany College of Pharmacy, and his Doctor of Pharmacy from Nova Southeastern University.  He holds faculty appointments with the University of Florida, Nova and Palm Beach Atlantic Colleges of Pharmacy. He also is the national training coordinator for the US Army Reserve Junior Pharmacy Officer Program.

Linda B. Rolston, Pharm.D., C.G.P., is the Clinical Pharmacy Specialist for Long Term Geriatric Care. She holds academic appointments as Clinical Assistant Professor at the University of Florida, Nova Southeastern University College of Pharmacy, Florida A&M, PBA (Palm Beach Atlantic), Mercer University School of Pharmacy and SUNY (Buffalo). Dr. Rolston received her B.S. in Pharmacy and her Doctor of Pharmacy from the University of Florida. She completed her Certification in Geriatrics with AACP in 1997. She is an active member of ASCP, FPA, FSHP, SWFSHP, Pinellas Pharmacy Association, and Pinellas County Gator Boosters.

Victor F. Rozance, Jr., B.S., Pharm.D., is the Clinical Pharmacy Specialist in Critical Care. He holds academic appointments as Clinical Instructor at the University of Florida, Nova Southeastern University College of Pharmacy, and Mercer University School of Pharmacy. Dr. Rozance received his B.S. in Pharmacy from Ohio State University and his Doctor of Pharmacy from University of Florida. He is a certified ACLS instructor and Cardiac Rehabilitation Pharmacist. Research includes weight-based heparin dosing in the veteran population with MI and acute angina, Co-investigator for "Comparative Toxicity of Gentamicin ODA & TDA in Surgical vs Medical ICU."

Devin Salter, Pharm.D., is a Clinical Pharmacist rotating to different areas of pharmacy practice. She received her Doctor of Pharmacy from University of Florida and completed a Pharmacy Practice Residency at Bay Pines VAHCS. Her areas of interest include Internal Medicine, Intensive Care, and Infectious Disease. Her responsibilities include training new pharmacy staff and residents on inpatient order entry as well as the Bar Code Medication Administration program.

Marjorie Showalter, Pharm.D, CACP, is a clinical pharmacist at the Sarasota VA CBOC (Community Based Outpatient Clinic).  She holds academic appointments at University of Florida (UF), Nova Southeastern University, Palm Beach Atlantic University-Lloyd Gregory School of Pharmacy and Ohio Northern University.  Dr. Showalter received her Doctor of Pharmacy degree from Nova Southeastern University in 2002.  After graduating, she completed a Primary Care Residency at the Shands UF Eastside Community Practice in Gainesville, Florida. In 2007, she became a Certified Anticoagulation Care Provider. Dr Showalter's responsibilities include anticoagulation services, disease state management, including hyperlipidemia, diabetes mellitus and hypertension and conducts New Patient Orientation Group classes in Sarasota.  She currently provides diabetes counseling as part of the diabetes nutrition classes.  Dr. Showalter is a member of FSHP and the president elect of the Sarasota Regional Society of Health-System Pharmacists. She is also active in Kappa Psi Pharmaceutical Fraternity where she has served as a past national officer and served on several national committees.

Brian R. Steele, Jr., Pharm.D., BCPS, AE-C, CGP, is a Clinical Pharmacy Specialist in ambulatory care.  Dr. Steele earned his Doctor of Pharmacy degree in 2003 from Northeastern University in Boston, Massachusetts.  He was boarded as a Pharmacotherapy Specialist in 2006 and was certified as an Asthma Educator and a Geriatric Pharmacist in 2007.  Dr. Steele holds a certification in Advanced Cardiac Life Support.  He is a facilitator at the University of Florida’s College of Pharmacy in St. Petersburg.  At Bay Pines VAHCS, Dr. Steele assists pharmacy residents in research and serves on the Residency Advisory Committee, the Medical Use Evaluation Committee, and the Pharmacy Professional Standards Board.  He is a member of the Florida Society of Health-System Pharmacists and the Pinellas County Pharmacist Association.  His practice interests include asthma, diabetes and sports medicine.

Catherine Sypniewski, Pharm.D., is the Surgical Clinical Pharmacy Specialist for both inpatient and outpatient care. She holds academic appointments as Clinical Instructor for Nova Southeastern University College of Pharmacy and Mercer University School of Pharmacy. Dr. Sypniewski received a B.S. in Chemistry from Loyola University and her Doctor of Pharmacy from the University of Florida.

Steven Totterdale, B.S, Pharm.D., is a Clinical Pharmacist at the Sarasota VA Primary Care Clinic (CBOC).  He holds academic appointments at the University of Florida, Nova Southeastern University, Palm Beach Atlantic University-Lloyd Gregory School of Pharmacy.  Dr. Totterdale received his BS in Pharmacy from Ferris State University and his Doctor of Pharmacy degree from Nova Southeastern University.  He is a member of the Professional Standards Board at Bay Pines Medical Center.  Dr. Totterdale responsibilities include anticoagulation services, disease state management, including hyperlipidemia, diabetes mellitus and hypertension.  He is also the site coordinator for the Sarasota Clinic MOVE (Moving Overweight Veterans Everywhere) program.  Dr. Totterdale is a member of FSHP and is a past president of the Sarasota Regional Society of Health-System Pharmacists.

J. Mark Trotter, Pharm.D., C.G.P., is the Clinical Pharmacy Specialist in Internal Medicine. He holds academic appointments as Clinical Instructor at the University of Florida, Nova Southeastern University College of Pharmacy, and Mercer University School of Pharmacy. Dr. Trotter received his Pharm.D. and B.S. in Pharmacy from University of Georgia. He also completed a residency in Pharmacy Practice at the University Medical Center in Jacksonville. Dr. Trotter completed his Certification in Geriatrics with CCGP in 1999, and recertified in 2003.

Trista Wagoner, Pharm.D., is a clinical pharmacy specialist in the Emergency Department.  She works with the emergency medicine team reviewing medication orders, counseling patients, and aiding in emergency situations.   Dr. Wagoner’s other practice interests include infectious disease and cardiology. Dr. Wagoner received her Pharm.D. from the Ohio Northern University. She completed a post-doctoral residency in Pharmacy Practice resident at Bay Pines VAHCS.  Her residency research project was a retrospective outcome evaluation of outpatients with community acquired pneumonia treated with various antibiotics.

Camaro West, B.S., Pharm.D., BCPS, CACP, is the Outpatient Pharmacy Program Manager. Dr. West received her B.S. in Pharmacy from Auburn University and her Doctor of Pharmacy from the University of Florida. She is board certified in Pharmacotherapy and is a Certified Anticoagulation Provider. Her research interests are in endocrinology with emphasis on Type-2 Diabetes.

John Winiecki, Pharm.D., is the Assistant Pharmacy ADPAC for the Department of Pharmacy. Dr. Winiecki received his Doctor of Pharmacy from Nova Southeastern University in 1998 and completed a Pharmacy Practice Residency at Bay Pines VAHCS in 1999. He previously completed a research project on an Evaluation of Serum Cholesterol Levels in HIV Positive Patient's Receiving Protease Inhibitors. His clinical practice interests include Internal Medicine, HIV/AIDS, and Mental Health.
