  1. Please complete the Declaration for Federal Employment and the Application for Associated Health Occupations forms and mail the completed paper copies of both to Carolyn Stephens by January 5th, 2009.

  2. Complete an electronic copy of the Bay Pines VA Healthcare System residency program application (“Application No 3 ”). Please then email the “Application No 3” along with a separate letter with your discussion of your professional goals and objectives, and your CV to Carolyn Stephens by January 5th, 2009 (do not mail paper copies of these).
  3. Contact your college of pharmacy to have a paper copy of your college transcripts mailed to Carolyn Stephens by January 5th, 2009. We would suggest that you contact your institution several weeks prior to the due date to ensure timely delivery.
  4. Please have 3 letters of recommendation emailed to Carolyn Stephens by January 5th, 2009. You can email the form to the individuals that will complete them and then have them email it directly to Carolyn Stephens from their email address using the attached form (do not send paper copies).
